Illinois Agencies: April 2026 EVV Deadlines Are Coming

Illinois Agencies: April 2026 EVV Deadlines Are Coming

The landscape of home health care in Illinois is shifting. For Home Health Care Services (HHCS) provider agencies, a major regulatory change is on the horizon. Starting April 1, 2026, new Electronic Visit Verification (EVV) policies will officially take effect.

If your agency bills State Plan Medicaid, HealthChoice Illinois (HCI) Managed Care Organizations, or FIDE SNP MCOs, now is the time to prepare.

Here is exactly what you need to know about the new mandates and how to ensure your agency stays compliant without disrupting your daily operations.

The New Reality: EVV is Mandatory

The Illinois Department of Healthcare and Family Services (HFS) has updated its policy to align with both State and Federal requirements. The core of this update is simple: accurate, electronic verification of service delivery is no longer optional — it is essential.

GPS Location Tracking is Required

Under the new policy, agencies must use a mobile application for visit verification. This isn’t just about clocking in and out. The system must capture GPS location at the start and end of every service delivery.

To do this, you should use a qualified third-party EVV vendor that integrates with the state system.

The 75% Compliance Threshold

The state isn’t just asking you to implement a system; they are measuring how well you use it. HFS will monitor compliance on a quarterly basis based on the State Fiscal Year (SFY).

Agencies must meet a minimum compliance threshold of 75% each quarter.

This score is calculated based on visit data from the start to the end of the quarter. It is important to note that your score does not roll over. If you have a great first quarter but slack off in the second, your previous success won’t save you from penalties. Every quarter is a fresh start — and a fresh test.

The Cost of Non-Compliance

The state has outlined a clear, three-strike structure for agencies that fail to meet the 75% threshold, fail to use mobile apps, or fail to track caregiver Social Security Numbers properly.

Here is what happens if your agency falls short:

  • First Occurrence: Your agency administrator(s) will be required to complete Learning Management System (LMS) modules within 30 days. This is a warning shot — a mandatory re-education to get you back on track.
  • Second Occurrence: The stakes get higher. You will be forced to submit a formal Compliance Action Plan (CAP) to the state within 10 business days. This creates significant administrative burden and puts your agency under a microscope.
  • Third Occurrence: This is the scenario you want to avoid at all costs. If you fail a third time, your agency will be referred to the HFS Office of Inspector General (OIG) for investigation.

An OIG investigation is serious business. It can damage your reputation, disrupt your business, and potentially threaten your ability to operate.

Why You Should Start Preparing Now

April 1, 2026, is the effective date, but waiting until March 2026 to implement a solution is a recipe for disaster.

Transitioning to a new technology takes time. Your caregivers need to learn the mobile app. Your administrative staff needs to understand how to monitor compliance dashboards. You need to iron out bugs and ensure that GPS tracking works reliably in the field.

If you wait until the deadline, you risk hitting that first quarter with low compliance scores simply because your team is still on the learning curve.

The good news is that GEOH has the fastest onboarding process in the market (2 weeks or less!) AND will train your new caregivers on how to use the app every Monday!
